Utah Immunization Requirements
In order to attend Tonaquint Intermediate School, a student must have proof of receiving the following immunizations:Grade 6
5 DTP(Diptheria, Tetanus, Pertussis)- 4 if the 4th dose given after the 4th birthday
4 Polio (IPV)- 3 if the 3rd dose given after the 4th birthday
2 Measles (MMR)- the first dose must be given on or after the 1st birthday.
1 Mumps (included in MMR)
1 Rubella (included in MMR)
3 Hepatitis B
Grade 7
A student entering 7th Grade must also have proof of receiving the following immunizations:
3 Hepatitis B
1 Tetanus/Diphtheria (Td) booster
1 Varicella (chickenpox) - Previous history of chickenpox is OK; a parent must sign verification statement on school immunization record.
5 DTP/DTaP/DT - 4 doses
4 Polio
2 Measles
1 Mumps
1 Rubella
A student born AFTER July 1, 1996
5 DTP/DTaP/DT - 4 doses if 4th dose was given on/after the 4th birthday
4 Polio - 3 doses if 3rd dose was given on/after the 4th birthday
2 Measles
1 Mumps
1 Rubella
3 Hepatitis B
1 Varicella (chickenpox) - history of disease is acceptable, parent must sign verification statement
2 Hepatitis A
PLEASE NOTE:
As of May 7, 2007 all Utah students, kindergarten through grade 12, are now required to have two doses of the Measles, Mumps, Rubella (MMR) vaccine, rather one dose of the MMR and one additional dose of the measles vaccine.
